I agree to release and indemnify NCC Cricket Academy from all liabilities arising from participation in programs and events. I agree to the Rules and Policies as a member of the NCC Cricket Academy (required)

MEMBERSHIP TERMS & CONDITIONS WAIVER & RELEASE Acknowledgment of Risks, Injury & Obligations - I acknowledge that the activity I am (or my child) to undertake is a dangerous activity and that by participating in it I am exposed to certain risks. I acknowledge and understand that whilst participating in such activity: My child or children may be injured, physically or mentally, or may die; My child or children s personal property may be lost or damaged; Other persons participating in such activity may cause my child or children injury or may damage my property. My child or children may cause injury to other persons or damage their property The conditions in which the activity is conducted may vary without warning. My child or children may be injured or die or suffer damage to their property as a result of the negligence or breach of contract of NCC Cricket Academy. There may be no or inadequate facilities for treatment or transport of my child or children if they are injured I assume the risk of and responsibility for any injury, death or property damage resulting from my child or children s participation in the activity.

RELEASE AND IDEMNIFY I (or my child) participates in the activity at my sole risk and responsibility. I release, indemnify and hold harmless NCC Cricket Academy, its servants and agents, from and against all and any actions or claims which may be made by me or on my behalf or by other parties for or in respect of or arising out of any injury, loss, damage or death caused to me or my property whether by negligence, breach of contract or in any way whatsoever.

ADMINISTRATION Appropriate covered footwear & a shirt must be worn at all times while in the facility. All weights and equipment must be put back after use. Photo ID cards (i.e. membership card or drivers license) must be carried and shown upon request. Shared facility access with a non-member will result in forfeiture of membership effective immediately Each member must respect Officials, Coaches, members and other facility users and behave in an appropriate manner at all times NCC Cricket Academy Staff reserves the right to rescind the rights of members not complying with the terms and conditions of the membership Members may not participate in other Niagara leagues and tournaments (sanctioned or affiliated with the City) Members under the age of 16 must be accompanied by an adult at all times within the facilities (Indoor/Outdoor). If purchasing personal training sessions or High Performance group sessions, I understand that the sessions and/classes are non-refundable and non-transferable I have read and agree with all of the above and understand that the above applies to all NCC Cricket Academy, its servants and agents, locations Permission to use photographic images By enrolling in this program, I allow NCC Cricket Academy, its servants and agents, to use my name and photographs that may be used in various NCC Cricket Academy communications including the newsletter and website. I understand group photographs taken at NCC Academy events may be used without identifying individual members.

SECTION 3 MEMBERS DECLARATION & PAYMENT DETAILS Before signing this document, I have read, understand and hereby agree to the terms and conditions of membership as defined in Section 2 of this membership form and know that it affects my legal rights. I agree to pay the following. I understand the fees current and advance are non-refundable and only creditable toward future programs if I decide to place membership on hold or withdraw.
